What to Bring

The following items are necessities when coming in for your first appointment:

  • Full Name / Proper ID 
  • Medical Card (if you have insurance)
  • Any court-related documentation (if applicable) 
  • Referral source contact information (for example: caseworkers, court authority)
  • List of medicines you may be taking
