Call to Make an Appointment

To make your first appointment for any of our program services please contact our main desk. For further information about program enrollment contact Katherine Vega at 216-634-3269 ext 102.

Please Note: Appointments are for individuals in the process of enrollment only. If you are a probation / parole officer, court authority, or case manager, and you would like to make a referral, please click here.

What to Bring

  • Full Name / Proper ID
  • Medical Card (if you have insurance)
  • Any court-related documentation (if applicable)
  • Referral source contact information (for example: caseworkers, court authority)
  • List of medicines you may be taking

What to Expect

  • After requesting an appointment, someone will be in contact within the next 24-48 hours.
  • Accepted requests will be followed up by a thirty minute pre-assessment set up to determine a possible level of care. For more information, see the Treatment Services tab.
